A Valuable Service for Adults with Special Needs
Johnson Adult Foster Care (JAFC) is designed around evidence based treatment, led by valid and reliable assessment measures and research on the needs of the clients. JAFC serves adults from 18 to 99 years of both genders. JAFC will accept any combination of the following special needs:
1. Disorders on the Autism Spectrum
2. Intellectual deficits: mild, moderate, severe
3. Psychiatric Disorder, including schizophrenia
Services are designed from an initial comprehensive assessment of needs using tools recommended by the Ontario government for specific populations: interRAI for people with psychiatric illness, SIS for people with intellectual and developmental disabilities and the CARS for people with autism. There are three foundations to the JAFC treatment approach:
Needs First
The people we serve often have great difficulty performing such basic tasks as understanding instructions, communicating their needs, getting dressed and cleaning themselves. Many need help with incontinence, managing money and accessing the community. Our foster parents and staff assess the individual for basic human needs and design a plan to meet every identified need. Our goal is to ensure that every individual living in our homes feel deeply cared about and completely safe.

What Is Adult Foster Care?
Senior adult foster care, sometimes called "Homes Plus" at Johnson Adult Foster Care, is a sheltered living arrangement with certified, trained care providers in family homes. Typically, individuals living in an adult foster care setting have special needs that make it difficult for them to live alone.
Adult foster care in private homes is provided by individuals or families who live in the home. In these homes, residents are encouraged to participate in daily family activities.
Corporate adult foster care, on the other hand, is offered in a home-like setting, but staffed by paid employees. In both types of adult foster care, an adult caregiver must be present during normal sleeping hours.
Needs Assessment
Treatment planning and outcome evaluation are based on comprehensive needs assessment completed on clients shortly after admission and every year thereafter. The specific tool used for the needs assessment varies according to the core diagnosis of the individual.
Level of Support to carry out tasks of daily living
It is clear that the population of adults with intellectual disability in residential care present a significant demand on caregivers to meet basic human needs. JAFC provides this level of support in our four bed homes. Each home is led by a live-in couple with staff support on shift. The staff are trained and supported. The program is designed to meet to any combination of the adaptive behavioural needs listed above. Many of the deficits in "daily living skills" such as washing, dressing, using money and taking responsibility that is observed in the population of adults with intellectual disability is prevalent among populations with psychiatric disorders, such as schizophrenia, and among adults with autism. The skills required of staff and the resources needed to support the adult is similar regardless of the primary diagnosis that is leading to the deficits in daily living skills.

Positive Behavioural Supports
Some of the people we serve present serious chronic behaviour problems such as self abuse and aggression. They often display maladaptive habits and behaviour that serves as a barrier to involvement with family and community. JAFC staff and foster parents are trained in applied behaviour analysis, assessment instruments and interventions. The interventions provided by JAFC are designed:
a) to anticipate behaviour problems
b) to limit the opportunity to express the behaviour
c) to learn the meaning of the behaviour as communication
" where the behaviour has a function, teach the individual how to communicate his/her needs appropriately
d) to teach new behaviour through positive behavioural supports
